Objectives

The solution aims to:

  • Develop standardised electronic patient assessments for patient histories, risk assessments (falls, pressure areas, etc.) and patient outcome assessments (FIMs, Bartells, etc.)
  • Facilitate completion of a patient’s assessment via telephone prior to admission or at the time of the patient’s admission
  • Identify and report patient risks prior to admission, at the time of admission, during an episode of care and post discharge
  • Automatically generate a plan of action for each assessment according to the risks identified and the score achieved
  • Improve patient assessment processes so they are less time consuming and more accurate
  • Develop a record of all patient assessments, which can be viewed online at any stage of a patient’s episode of care (pre-admission, through the inpatient phase and post discharge)
  • Provide a comprehensive summary of patient risks identified through a range of assessments

  • Audit reports identifying compliance to assessment completion for all stages of an episode of care
  • Action plans for each assessment according to the risks identified and the score achieved

All TrendCare reports and graphs can be saved as PDF, text or CSV format e.g. for transfer into a spreadsheet.
