Objectives

• To improve patient assessment processes so that they are less time consuming and more accurate.
  
• To develop standardised electronic patient assessments for patient histories, risk assessments (falls, pressure areas, etc) and patient outcome assessments (FIMs, Bartells, etc).
  
• To identify and report patient risks prior to admission, at the time of admission, during an episode of care and post discharge.
  
• To develop a record of all patient assessments which can be viewed online at any stage of a patient’s episode of care (pre-admission, through the inpatient phase and post discharge).
  
• To provide a comprehensive summary of patient risks identified through a range of assessments.
